Body temperature is a fundamental physiological parameter representing the internal thermal state of an organism, typically maintained within a narrow range (normothermia) through the process of thermoregulation (StatPearls, 2023). It is not a discrete molecular target such as a receptor or enzyme, but rather a systemic manifestation of metabolic activity and heat exchange regulated by the preoptic area of the hypothalamus (Britannica, 2023). In clinical contexts, deviations in body temperature like fever (pyrexia) are often mediated by pyrogens that induce the synthesis of Prostaglandin E2, which subsequently resets the hypothalamic thermostat to a higher level (Mayo Clinic, 2023). Pharmacological interventions do not target "body temperature" directly but instead act upon the molecular pathways that regulate it, such as the inhibition of cyclooxygenase enzymes by antipyretics like ibuprofen or aspirin (NIH, 2021). Monitoring body temperature is essential for diagnosing infections, inflammatory conditions, and heat-related illnesses, making it a vital sign in medical practice. Excessive suppression of fever can sometimes mask underlying clinical worsening, while extreme temperatures (hypothermia or hyperthermia) can lead to organ failure and death (StatPearls, 2023).
Drugs modulate body temperature by targeting the biochemical pathways of thermoregulation, most commonly through the inhibition of cyclooxygenase (COX) enzymes to reduce Prostaglandin E2 (PGE2) production in the hypothalamus, thereby lowering the thermoregulatory set point during fever (StatPearls, 2023). Additionally, drugs like dantrolene act on ryanodine receptors in skeletal muscle to prevent heat production in conditions like malignant hyperthermia (NIH, 2022).
