Bone mineral density (BMD) is a physiological measurement reflecting the concentration of minerals, primarily hydroxyapatite, within bone tissue (NIH, 2023). It is typically assessed via dual-energy X-ray absorptiometry (DXA) and is expressed as grams of mineral per square centimeter (areal BMD) or cubic centimeter (volumetric BMD) (WHO, 2024). While BMD is a critical clinical endpoint and a proxy for skeletal strength, it is not a biological target molecule such as a receptor, enzyme, or transporter; rather, it is a complex phenotypic trait (StatPearls, 2023). Therapeutic interventions for metabolic bone diseases, such as bisphosphonates or monoclonal antibodies, aim to increase BMD by modulating the activity of bone-forming osteoblasts or bone-resorbing osteoclasts (PubMed, 2022). Consequently, BMD serves as a primary biomarker for diagnosing osteoporosis and monitoring the efficacy of pharmacological treatments in clinical practice (Journal of Bone and Mineral Research, 2021). Because it does not account for bone microarchitecture or geometry, BMD is often combined with clinical risk factors to predict fracture risk more accurately (Mayo Clinic, 2023).
