Overview

The bone extracellular matrix (BECM) is a specialized, dynamic structure composed of an organic framework, primarily Type I collagen, and an inorganic mineral phase consisting of hydroxyapatite crystals [PMID: 27533513]. It provides the essential mechanical strength and rigidity required for skeletal support and the protection of internal organs [StatPearls: NBK535448]. Beyond its structural role, the BECM serves as a critical reservoir for calcium and phosphate ions, maintaining systemic mineral homeostasis, and sequesters various growth factors that regulate bone remodeling [PMID: 30630011]. In pathological conditions such as osteoporosis and bone metastasis, the balance of matrix turnover is disrupted, leading to increased skeletal fragility [PMID: 29103323]. Therapeutic agents like bisphosphonates target the BECM by binding with high affinity to hydroxyapatite, thereby inhibiting osteoclast-mediated resorption [PubChem: CID 2088]. Additionally, bone-seeking radiopharmaceuticals utilize the matrix's mineral component to deliver targeted radiation to malignant bone lesions [PMID: 24037925].
