The bone marrow-derived mesenchymal stem cell (BM-MSC) secretome represents the complete set of bioactive factors secreted by BM-MSCs, including proteins, lipids, and nucleic acids often encapsulated within extracellular vesicles (Vizoso et al., 2017). This secretome acts as a mediator of paracrine signaling, influencing a wide array of cellular and molecular pathways rather than targeting a single receptor (Gnecchi et al., 2016). Its primary biological functions include immunomodulation, promotion of angiogenesis, and the inhibition of apoptosis, which collectively facilitate tissue repair and regeneration (Teixeira et al., 2013). In clinical contexts, the BM-MSC secretome is explored as a cell-free therapeutic strategy for treating inflammatory diseases, myocardial infarction, and neurodegenerative disorders (Vizoso et al., 2017). While it offers advantages over live cell transplantation, such as reduced immunogenicity and easier storage, it is not a single molecular target but a complex mixture of therapeutic agents. Consequently, its pharmacological application involves the simultaneous activation of multiple pathways, such as the Wnt/beta-catenin and TGF-beta signaling cascades, to achieve therapeutic outcomes (Gnecchi et al., 2016).
