Bone microstructure refers to the complex three-dimensional arrangement of trabecular plates and rods and the organization of cortical bone at the microscopic level. It is a fundamental determinant of bone strength and fracture resistance, providing the mechanical framework necessary for load-bearing and protection of internal organs [1]. Rather than being a single molecular target like a protein or enzyme, bone microstructure is a physiological endpoint that reflects the cumulative history of bone remodeling and mineral homeostasis [2]. In diseases such as osteoporosis, the deterioration of this microstructure leads to increased skeletal fragility and an elevated risk of fractures [3]. Clinical interventions aimed at improving bone health, including bisphosphonates, RANKL inhibitors, and parathyroid hormone analogs, work by influencing the cellular pathways that govern bone formation and resorption to preserve or enhance microstructural integrity [4,5]. Advanced imaging techniques are increasingly used to assess microstructural quality to better predict fracture risk beyond traditional bone mineral density measurements.
Bone microstructure is indirectly modified by pharmacological agents that modulate bone remodeling; anti-resorptive agents inhibit osteoclast-mediated bone breakdown to preserve existing architecture, while anabolic agents stimulate osteoblast activity to increase trabecular thickness and connectivity.
