Overview

"Bowel Qi" is a fundamental concept in Traditional Chinese Medicine (TCM) that refers to the physiological functional activities and movement (Qi) of the intestinal tract. It is not a specific molecular target, such as a receptor or enzyme, but rather a functional state characterizing intestinal conduction and the descending movement of waste [1, 4]. When Bowel Qi is obstructed, a pattern known as "Bowel-Qi obstructed syndrome" occurs, typically manifesting as constipation, abdominal distention, and systemic inflammation [1, 2]. This syndrome is frequently observed in acute clinical conditions, including stroke (acute cerebral infarction) and severe respiratory infections like COVID-19, where it is thought to exacerbate disease progression through the gut-lung or gut-brain axes [3, 8]. Therapeutic intervention for Bowel Qi obstruction involves the use of TCM formulas like Zhishi Rhubarb Soup or Dachaihu decoction, which aim to "unblock" the bowels and restore Qi flow [1, 5]. Modern network pharmacology suggests that these treatments interact with multiple molecular pathways, particularly those involving inflammatory cytokines such as TNF-α and IL-6, and the regulation of the enteric nervous system to promote motility [1, 3]. Consequently, while Bowel Qi remains a clinical and physiological concept in holistic medicine, its therapeutic modulation correlates with the regulation of systemic immune responses and metabolic homeostasis [4, 7].
