BPI fold-containing family B member 2 (BPIFB2) is a secreted protein that belongs to the BPI fold protein superfamily, specifically the BPIFB group, characterized by a distinctive BPI fold—a structural motif with two domains in a boomerang shape that creates hydrophobic binding pockets[1][2][6]. BPIFB2 is primarily involved in the innate immune response, acting at the mucosal surfaces of the mouth, nasal cavities, and lungs, where it is highly expressed in salivary glands, esophagus, and hypertrophic tonsils[1][2][3]. It can bind lipids and lipopolysaccharide molecules, similar to other family members, and is thought to play a role in host defense against bacterial pathogens. Reduced BPIFB2 expression is associated with chronic rhinosinusitis, suggesting a role in mucosal immunity dysfunction[1][2]. While BPIFB2 is linked to certain genetic and inflammatory diseases, there are currently no drugs targeting this protein, and it is not considered a receptor, enzyme, or classical therapeutic target in clinical practice[2][6][7].
