Overview

Brain metabolism modulation refers to the therapeutic manipulation of energy-producing processes within the central nervous system to address bioenergetic deficits associated with neurological and metabolic disorders [1, 2]. It is not a discrete molecular target, such as a single receptor or enzyme, but a broad physiological strategy targeting pathways related to glucose transport, mitochondrial function, and alternative fuel utilization [2, 4]. Chronic impairment in brain glucose uptake and mitochondrial efficiency is a hallmark of neurodegenerative diseases like Alzheimer's and Parkinson's, where metabolic decline often precedes clinical symptoms [1, 4]. Therapeutic approaches to modulate brain metabolism include pharmacological agents like GLP-1 receptor agonists and SGLT2 inhibitors, which improve insulin sensitivity and glucose utilization [4]. Additionally, non-pharmacological methods such as deep brain stimulation (DBS) and ketogenic diets are explored to bypass glucose processing defects or stimulate regional metabolic activity [7, 10, 11]. By restoring metabolic homeostasis, these strategies aim to enhance neuroplasticity, reduce neuroinflammation, and preserve cognitive and motor functions in affected patients [5, 6, 11].
