Overview

PGT121 is a broadly neutralizing human monoclonal antibody targeting the V3-glycan-dependent supersite within the HIV-1 envelope protein gp120. It was originally isolated from an individual with potent anti-HIV serum. PGT121 effectively neutralizes a high proportion of HIV-1 strains (64-70%), primarily by binding adjacent to the N332 glycan in the gp120 V3 loop and allosterically interfering with gp120-CD4 receptor engagement, thereby blocking viral entry. It has shown safety and antiviral efficacy in non-human primates and human phase I trials, where it lowers viral load and, in some cases, induces sustained viral suppression in HIV-1-infected individuals. Combination therapy with other bnAbs is being investigated to minimize resistance. Glycoengineering can optimize Fc-mediated functions such as ADCC. PGT121 is not a naturally occurring molecular target, but rather a biologic drug.

Mechanism of action

Binds to the V3-glycan-dependent site on HIV-1 envelope protein gp120. Blocks gp120 binding to the CD4 receptor, preventing viral entry. Neutralizes diverse HIV-1 strains with high potency. Engages Fcγ receptors (depending on glycoengineering) for ADCC.
