Bulk extracellular fluid (ECF) represents the total volume of body water located outside of the cell membranes, including the interstitial fluid that bathes tissues and the plasma within the circulatory system (Guyton and Hall, 2020). It serves as the primary medium for the exchange of nutrients, gases, and metabolic wastes between the blood and the intracellular environment, thereby maintaining the stability of the internal milieu (StatPearls, 2023). In pharmacology, the ECF is recognized as a non-molecular target for therapeutic agents that exert their effects through physical or chemical changes rather than by binding to specific receptors or enzymes (NCBI, 2022). For instance, osmotic diuretics such as mannitol are administered to increase the osmolality of the ECF, which draws water out of swollen tissues like the brain or eye (PubChem, 2024). Similarly, systemic alkalinizing agents like sodium bicarbonate are used to neutralize excess hydrogen ions in the ECF to treat metabolic acidosis (StatPearls, 2023). Because the ECF is a broad physiological compartment, drugs targeting it can significantly impact fluid balance and electrolyte concentrations, necessitating careful clinical monitoring (NIH, 2023).
