The bulk extracellular fluid and osmotic compartment encompass all body fluids found outside of cells, primarily consisting of interstitial fluid and blood plasma (StatPearls, 'Physiology, Extracellular Fluid'). This compartment plays a vital role in maintaining homeostasis by facilitating the transport of nutrients, gases, and metabolic waste products throughout the body. In a pharmacological context, it serves as a non-molecular target for osmotic agents, which exert their effects by altering the physical osmotic pressure of the fluid rather than binding to specific receptors (DrugBank, 'Mannitol'). By increasing the tonicity of the extracellular environment, these drugs induce the movement of water across semi-permeable membranes. This mechanism is therapeutically exploited to reduce intracranial and intraocular pressure, as well as to promote diuresis or bowel movements in clinical settings (NIH, 'Osmotic Diuretics').
Osmotic agents increase the solute concentration in the extracellular fluid, creating an osmotic gradient that draws water from the intracellular or interstitial space into the vascular or luminal space via osmosis (StatPearls, 'Osmotic Diuretics'; PubMed, PMID: 29071301).
