Target intelligence / Profile preview
Bulk tissue water and extracellular fluid (ECF) represent the aqueous environment surrounding cells and within the vascular system, serving as the primary solvent for all biochemical reactions and the medium for nutrient and waste transport (NIH, NBK541059). While not a traditional protein-based molecular target like a receptor or enzyme, these fluid compartments are the direct pharmacological targets for osmotic diuretics and related agents. These drugs, such as mannitol, work by increasing the osmotic pressure of the ECF, which facilitates the movement of water out of swollen tissues—such as the brain or eye—into the bloodstream to reduce life-threatening pressure. This mechanism is a cornerstone in the emergency management of cerebral edema and acute angle-closure glaucoma. Maintaining the precise volume and solute composition of these fluids is critical for cardiovascular stability, cellular integrity, and overall physiological homeostasis.
Osmotic agents increase the solute concentration and osmolality of the extracellular fluid, creating a physical osmotic gradient that draws water from the intracellular or interstitial compartments into the vascular space for subsequent renal excretion (StatPearls, NBK557526).
