Overview

Bulk water and blood plasma constitute the fundamental aqueous environments of the human body, serving as the primary solvent for biochemical reactions and the medium for systemic transport. Blood plasma is the extracellular fluid component of blood, containing water, electrolytes, proteins, and nutrients, and is essential for maintaining homeostasis, pH balance, and osmotic pressure (StatPearls, 2023). While not a specific molecular target like a protein or enzyme, these compartments are therapeutically manipulated by osmotic diuretics and volume expanders. For instance, mannitol increases the osmolarity of bulk water in the renal tubules to promote water excretion, while albumin and crystalloids are used to restore plasma volume in clinical settings such as hypovolemic shock (NIH, 2022). Because they lack specific binding sites, drug interactions with these media are governed by physical properties like osmotic pressure and oncotic pressure rather than biochemical signaling. Consequently, bulk water and plasma are categorized as physiological compartments or solvents rather than discrete therapeutic targets.
