Overview

Bulk water protons in the extracellular fluid serve as the fundamental signal source for Magnetic Resonance Imaging (MRI), rather than acting as a traditional therapeutic target like a receptor or enzyme (Source: StatPearls, MRI Physics). These protons are part of the water molecules (H2O) located in the interstitial and intravascular spaces. In clinical practice, they are the physical substrate for paramagnetic contrast agents, such as gadolinium-based contrast agents (GBCAs), which alter the magnetic properties of the protons to enhance image contrast (Source: NIH, National Institute of Biomedical Imaging and Bioengineering). By shortening the T1 and T2 relaxation times of these protons, contrast agents allow for the detection of physiological and pathological changes, including increased vascular permeability, inflammation, and tumor margins (Source: PubMed, PMID: 25650518). Consequently, while not a site for therapeutic intervention, bulk water protons are critical for diagnostic monitoring and patient selection in various disease states.
