Overview

Carbohydrate antigen 242 (CA242) is a sialylated carbohydrate epitope found on high-molecular-weight mucins, primarily associated with gastrointestinal malignancies such as pancreatic, colorectal, and gastric cancers (Nilsson et al., 1992, Cancer Res). It is chemically related to the sialyl-Lewis A (CA19-9) and sialyl-Lewis X structures but is uniquely defined by its recognition by the monoclonal antibody C242 (Johansson et al., 1991, Cancer Res). In healthy individuals, CA242 expression is minimal, but it becomes significantly elevated in the serum of patients with certain cancers, serving as a specific and sensitive biomarker for diagnosis and disease monitoring (He et al., 2018, OncoTargets and Therapy). Beyond its diagnostic utility, CA242 has been explored as a therapeutic target for antibody-drug conjugates (ADCs), such as cantuzumab mertansine (IMGN242), which aim to deliver potent cytotoxic agents directly to tumor cells expressing the antigen (Ritchie et al., 2007, Journal of Clinical Oncology). These therapeutic strategies rely on the high tumor-to-normal tissue ratio of CA242 expression to minimize systemic toxicity. However, challenges remain regarding the potential for low-level expression in normal biliary and intestinal tissues, which can impact the safety profile of CA242-targeted therapies (Smith et al., 2001, Clinical Cancer Research).
