Overview

The Carbonic anhydrase IX (CAIX) peptide-MHC complex is a specialized therapeutic target consisting of an intracellularly processed peptide fragment of the CAIX protein presented on the cell surface by Major Histocompatibility Complex (MHC) molecules, typically HLA-A*02:01. CAIX is a membrane-bound enzyme that plays a critical role in pH regulation and is highly overexpressed in various cancers, most notably clear cell renal cell carcinoma (ccRCC), as a response to tumor hypoxia. While traditional antibodies target the extracellular domain of the intact CAIX protein, targeting the pMHC complex allows for the use of T-cell receptor (TCR)-based or TCR-like (TCR-mimetic) strategies. This approach leverages the high specificity of T-cell recognition to direct potent immune responses against tumor cells. Because CAIX expression is minimal in most healthy tissues but nearly ubiquitous in certain solid tumors, this complex represents a high-value target for precision immunotherapy, including TCR-engineered T-cells and bispecific T-cell engagers.

Mechanism of action

Drugs targeting this complex, such as TCR-mimetic antibodies or TCR-engineered T-cells, bind specifically to the processed CAIX peptide fragment presented within the groove of the Major Histocompatibility Complex (MHC) on the tumor cell surface. This interaction triggers T-cell mediated cytotoxicity, leading to the selective lysis of CAIX-expressing malignant cells while potentially sparing normal tissues that do not present the specific peptide-MHC density required for activation.
