CEA and MUC-1 expressing tumor cells refers to a therapeutic strategy targeting two distinct tumor-associated antigens (TAAs) simultaneously. Carcinoembryonic antigen (CEA, or CEACAM5) is a cell surface glycoprotein involved in cell-cell adhesion and is highly expressed in most gastrointestinal, pancreatic, and lung carcinomas (PMID: 16533433). Mucin 1 (MUC-1) is a transmembrane glycoprotein that is normally expressed on the apical surface of ductal epithelia but becomes overexpressed and under-glycosylated in many cancers, including breast and ovarian cancer (PMID: 15105051). Targeting both antigens is intended to increase the breadth of the anti-tumor immune response and mitigate the risk of tumor escape due to antigen loss or heterogeneity. This dual-targeting approach is most notably utilized in viral vector vaccines like PANVAC, which also incorporate costimulatory molecules to enhance T-cell activation against these specific tumor markers (PMID: 19451554). These vaccines typically use viral vectors such as vaccinia or fowlpox to deliver the genetic sequences of the antigens directly to antigen-presenting cells. Once expressed, these antigens trigger a robust cytotoxic T-lymphocyte response aimed at destroying tumor cells that display both CEA and MUC-1 on their surface. Clinical trials have explored this approach in various solid tumors, demonstrating the potential for improved patient outcomes when combined with standard therapies.
The primary mechanism involves the presentation of CEA and MUC-1 peptides by antigen-presenting cells to stimulate the expansion of antigen-specific CD4+ and CD8+ T lymphocytes, which then recognize and eliminate tumor cells expressing these antigens.
