The carotid artery wall is a complex, multi-layered vascular structure consisting of the tunica intima, media, and adventitia, which provides structural integrity and regulates blood flow to the brain (StatPearls, 2023). It is a critical site for the pathogenesis of atherosclerosis, a condition characterized by the accumulation of lipids, fibrous elements, and inflammatory cells within the wall layers (NIH, 2022). This process can lead to carotid artery stenosis or the formation of unstable plaques, which are primary causes of ischemic stroke. Although the carotid artery wall is an anatomical structure and not a single molecular target, it is the physiological location where numerous drugs exert their effects. For instance, statins target HMG-CoA reductase within the wall's cellular environment to reduce lipid deposition, while antihypertensive agents like ACE inhibitors help prevent pathological wall remodeling (PubMed, 2021). Monitoring the wall's thickness and plaque stability is essential for assessing cardiovascular risk and the efficacy of these therapeutic interventions.
Pharmacological agents do not target the wall as a single molecule but rather act on specific molecular components within the tissue, such as HMG-CoA reductase to lower lipids, or angiotensin-converting enzyme to reduce wall tension and pathological remodeling.
