Overview

Caudal type homeobox protein 4 (CDX4) is a homeobox transcription factor expressed during vertebrate development. It integrates canonical Wnt signaling and acts in concert with other caudal-type homeobox family members to regulate posterior tissue patterning, extra-embryonic tissue formation, and blood cell specification by controlling HOX gene networks. In hematopoiesis, CDX4 is essential for the specification of hematopoietic progenitors; aberrant expression drives leukemogenesis in acute myeloid leukemia through feedback with HOX factors and cytokine hypersensitivity. CDX4 also influences the anterior-posterior location of foregut organs and, though less active than CDX2 in intestinal differentiation, has a modulatory role in colorectal cell differentiation. Knockout or compound mutant models reveal its critical role in caudal embryonic development, with loss leading to serious patterning, organogenesis, and hematopoietic defects. No direct clinical drugs target CDX4, but its pathway is implicated in cancer and congenital disorders. CDX4 is therefore a central developmental transcription factor whose dysregulation can contribute to malignant transformation, especially in hematological cancers.
