Overview

CD38 is a multifunctional glycoprotein expressed on immune cells. It functions as both a receptor for CD31 and an ectoenzyme that metabolizes NAD+ and NADP+, playing roles in cell adhesion, signal transduction, calcium signaling, and immune responses. High CD38 expression is associated with poor prognosis in CLL and can contribute to resistance against PD-(L)1 immunotherapies. It is a target for monoclonal antibody therapies in multiple myeloma and other hematologic malignancies.

Mechanism of action

Anti-CD38 monoclonal antibodies bind to CD38 on tumor cells, leading to cell death through multiple mechanisms, including antibody-dependent cell-mediated cytotoxicity (ADCC), complement-dependent cytotoxicity (CDC), and antibody-dependent cellular phagocytosis (ADCP). Small molecule inhibitors target the enzymatic activity of CD38, modulating NAD+ levels and related signaling pathways.
