Cellular proteins and glutathione refers to a critical intracellular system involved in the detoxification of reactive electrophilic metabolites and the maintenance of redox homeostasis. Glutathione (GSH) is a tripeptide (gamma-L-glutamyl-L-cysteinyl-glycine) that acts as a major endogenous antioxidant and a substrate for detoxification reactions [PubChem: CID 124886]. In the context of pharmacology, this system is most notably associated with the metabolism of drugs like acetaminophen, where the reactive metabolite N-acetyl-p-benzoquinone imine (NAPQI) is neutralized by conjugation with glutathione [StatPearls: NBK441917]. When glutathione stores are depleted, these reactive intermediates bind covalently to essential cellular proteins, particularly those within the mitochondria, leading to loss of enzymatic function, oxidative stress, and cellular necrosis [PubMed: 22403130]. Therapeutic agents such as N-acetylcysteine (NAC) are used to replenish glutathione stores and prevent this protein arylation [NIH: LiverTox]. This interaction is a fundamental mechanism in drug-induced liver injury and serves as a key consideration in drug safety and toxicology [PubMed: 15743955].
Reactive drug metabolites are detoxified through conjugation with glutathione; when glutathione is depleted, these metabolites bind covalently to cellular proteins, leading to cellular dysfunction and death.
