The central airway structure refers collectively to the main conducting passages of air within the respiratory system: primarily the trachea and mainstem bronchi. These structures are responsible for transporting air from outside the body into and out of each lung. The trachea is supported by C-shaped cartilaginous rings that prevent collapse during breathing. The epithelium lining these airways contains mucus-secreting goblet cells and ciliated cells that help trap and remove pathogens and particles from inhaled air through mucociliary clearance. The central airways are critical sites for certain pathologies such as malignant obstruction due to tumors like squamous cell carcinoma or small-cell lung cancer. Obstruction can be intrinsic, extrinsic, or mixed depending on whether tumor growth occurs within or around these passages. While not a molecular target per se, interventions targeting diseases of this region—such as stenting or laser therapy—are important in clinical management.
