Central analgesic pathways refer to the complex neural circuits within the brain and spinal cord responsible for the endogenous modulation and suppression of pain signals. A primary component is the descending inhibitory pathway, which originates in higher brain centers such as the periaqueductal gray (PAG) and the rostral ventromedial medulla (RVM), projecting down to the dorsal horn of the spinal cord (StatPearls, 2023). These pathways utilize various neurotransmitters, including endogenous opioids, serotonin, and norepinephrine, to attenuate the transmission of nociceptive information from peripheral nerves to the brain (Nature Reviews Neuroscience, 2009). Dysfunction in these pathways is a hallmark of chronic pain states, where a failure of endogenous inhibition or an increase in descending facilitation leads to hypersensitivity (Journal of Clinical Investigation, 2010). Pharmacological intervention often targets specific receptors within these pathways, such as opioid receptors or adrenergic receptors, to enhance inhibitory control and provide relief from acute and chronic pain (Mayo Clinic, 2024).
