Central memory T cells (Tcm) are a long-lived subset of memory T lymphocytes that play a critical role in long-term adaptive immunity. They are distinguished from other memory subsets by the expression of lymph node-homing receptors, specifically CCR7 and CD62L (L-selectin), which allow them to circulate through secondary lymphoid organs rather than peripheral tissues [4, 17]. Tcm cells possess a high capacity for self-renewal and rapid proliferation upon antigen re-exposure, primarily producing IL-2 to drive the expansion of effector T cell populations [11, 17]. In the context of cancer immunotherapy, Tcm cells are highly valued for their superior persistence and anti-tumor efficacy compared to more terminally differentiated effector cells, often serving as the preferred starting population for CAR-T cell manufacturing [3, 12, 15]. Conversely, in autoimmune conditions such as multiple sclerosis, Tcm cells can act as a reservoir for pathogenic auto-reactive clones, making them targets for sequestration or depletion by drugs like fingolimod or alemtuzumab [2]. Pharmacological modulation of Tcm cells typically involves manipulating cytokine signaling (e.g., IL-7, IL-15) or metabolic pathways like mTOR to enhance their survival or prevent exhaustion [11, 12].
