The central nervous system (CNS) cough center pathways comprise a network of neurons in the brainstem, primarily located within the medulla oblongata, that integrate sensory inputs to produce the cough reflex (StatPearls: Physiology, Cough). The primary relay station is the nucleus tractus solitarius (NTS), which receives afferent signals from the vagus nerve and communicates with a central pattern generator to coordinate the motor components of coughing (Canning et al., 2014). This pathway is the site of action for centrally acting antitussive drugs, such as dextromethorphan and codeine, which elevate the cough threshold by modulating various receptors including mu-opioid, sigma-1, and NMDA receptors (Bolser, 2006). Dysfunction or hypersensitivity within these central pathways is a significant factor in the development of chronic cough syndromes (Polverino et al., 2012). However, because these circuits are anatomically close to centers controlling breathing and consciousness, pharmacological modulation often results in side effects like sedation or respiratory depression. Modern research also highlights the role of the paratrigeminal nucleus and higher cortical centers in the voluntary control and suppression of cough (Mutolo, 2017). Understanding these pathways is crucial for developing non-opioid antitussives that lack the adverse effects associated with traditional central suppressants.
Centrally acting antitussives suppress the cough reflex by increasing the activation threshold of the medullary cough center and inhibiting excitatory neurotransmission within the nucleus tractus solitarius and associated brainstem circuits.
