The central nervous system pain-modulating pathways comprise a sophisticated network of neural circuits responsible for the endogenous regulation of nociceptive input (StatPearls, 2023). This system includes ascending pathways that transmit sensory information to the brain and descending pathways, primarily involving the periaqueductal gray (PAG) and rostral ventromedial medulla (RVM), which exert top-down control over spinal cord signaling (Nature Reviews Neuroscience, 2014). These pathways utilize various neurotransmitters, including endogenous opioids, serotonin, and norepinephrine, to either suppress (antinociception) or enhance (pronociception) pain perception (PubMed, 2019). In pathological states such as chronic or neuropathic pain, these pathways often become dysfunctional, leading to hyperexcitability and impaired inhibition (NIH, 2022). Therapeutic strategies aim to harness these pathways using analgesics like opioids, antidepressants, and anticonvulsants to restore the balance of pain modulation (Journal of Neuroscience, 2020).
Drugs modulate these pathways by activating descending inhibitory circuits (e.g., via mu-opioid receptors in the PAG) or enhancing the synaptic concentration of inhibitory neurotransmitters like norepinephrine and serotonin in the spinal dorsal horn (StatPearls, 2023; PubMed, 2021).
