Overview

Central nervous system (CNS) pain pathways refer to the neural circuits and molecular mechanisms within the CNS that transmit, process, and modulate pain signals originating from peripheral tissues. These pathways are responsible for both the sensory-discriminative (location, intensity) and affective-motivational (emotional response) aspects of pain. Key components include primary afferent neurons, spinal cord dorsal horn, ascending tracts (spinothalamic, spinoreticular), thalamus, somatosensory cortex, limbic structures, and descending modulatory pathways. Molecular mechanisms involve neurotransmitters, neuropeptides, ion channels, and G protein-coupled receptors. Alterations in these pathways contribute to chronic pain syndromes, and many analgesics target specific molecules within these circuits to modulate pain processing.
