Overview

Central neural pathways refer to the complex networks of neurons and their axonal projections within the brain and spinal cord that facilitate the transmission of electrical and chemical signals. These pathways, including the corticospinal tract for motor control and the spinothalamic tract for sensory input, are essential for coordinating all bodily functions, from basic reflexes to higher-order cognition (StatPearls, 2023). While not a single molecular target, these pathways contain numerous receptors, ion channels, and enzymes that are the focus of pharmacological intervention for various neurological and psychiatric conditions. Dysfunction within these pathways is central to the pathology of diseases such as Parkinson's, Alzheimer's, and multiple sclerosis (NIH, 2024). Drugs targeting components within these pathways must often cross the blood-brain barrier to exert their effects, presenting a significant challenge in drug development. Because the term encompasses a vast array of distinct molecular targets and anatomical structures, it is generally considered too broad for specific therapeutic classification.

Mechanism of action

Drugs do not target central neural pathways as a single entity; instead, they modulate specific receptors, transporters, or ion channels located within these pathways to alter neuronal signaling and circuit activity (StatPearls, 2023).
