The central pattern generator of swallowing is a specialized network of interconnected neurons, including both interneurons and motor neurons, predominantly located in specific brainstem nuclei—the dorsal swallowing group (DSG) in the nucleus tractus solitarii (NTS) and the ventral swallowing group (VSG) in the ventrolateral medulla. The DSG is crucial for timing and initiation of the swallowing pattern, while the VSG organizes motor output to relevant muscle groups. This circuit receives input from both peripheral sensory afferents and higher cortical areas, allowing adaptation of the swallowing pattern to the size and type of bolus. The CPG for swallowing underlies coordinated, rhythmic contraction of over 25 muscle pairs necessary for safe and effective transit of food and liquids through the oropharynx and esophagus. Failures or dysfunctions of this system manifest clinically as neurogenic dysphagia and are implicated in various neurological diseases. This is not a canonical molecular target for drug discovery but is a critical physiological control system.
