Centruroides neurotoxins are a group of potent, low-molecular-weight peptides found in the venom of scorpions from the genus Centruroides, such as the Arizona bark scorpion (Centruroides sculpturatus) (NIH, 2024). These peptides primarily target voltage-gated sodium (Nav) and potassium (Kv) channels in the nervous system, altering their gating properties and leading to excessive neuronal firing and a massive release of neurotransmitters (MDPI Toxins, 2024). This physiological disruption results in a severe clinical syndrome known as scorpionism, which can manifest as neuromotor hyperactivity, respiratory distress, and cardiovascular instability (NEJM, 2009). The primary therapeutic approach for severe envenomation is the use of specific antivenoms, such as Anascorp, which contain F(ab')2 antibody fragments that bind and neutralize the toxins in the bloodstream (FDA, 2011). Beyond their role in envenomation, these toxins are highly valued as molecular tools for studying ion channel function and are being explored as templates for the development of novel analgesics and insecticides (Toxicon, 2020). Their high specificity for particular ion channel isoforms makes them promising candidates for targeted drug design in treating various channelopathies (NIH, 2024).
The therapeutic mechanism involves the administration of antivenom containing F(ab')2 antibody fragments that specifically bind to and neutralize circulating neurotoxic peptides (NEJM, 2009). This sequestration prevents the toxins from interacting with their target voltage-gated ion channels on excitable membranes, thereby halting the progression of neurotoxic symptoms and facilitating the clearance of the toxin-antibody complexes from the systemic circulation (MDPI Toxins, 2024).
