Ceramide 6-II, also known as Ceramide AP, is a specific sphingolipid found in the stratum corneum of the human skin, consisting of an alpha-hydroxy fatty acid linked to a phytosphingosine base (Motta et al., 1993, Biochim Biophys Acta). It plays a crucial role in maintaining the skin's permeability barrier and regulating transepidermal water loss (TEWL) (Coderch et al., 2003, Am J Clin Dermatol). In dermatological conditions such as atopic dermatitis and psoriasis, the levels of Ceramide 6-II are often significantly reduced, leading to impaired barrier function and increased skin sensitivity (PMID: 12558444). While not a traditional protein target like a receptor or enzyme, it is a key therapeutic component in topical formulations designed to restore the lipid barrier and treat inflammatory skin disorders (PubChem). Its primary function is structural, contributing to the lamellar phases that prevent dehydration and protect against environmental pathogens. Therapeutic intervention typically involves the topical application of ceramide-dominant mixtures to replenish the depleted lipid pool (NIH). This approach helps to normalize the skin's physiological state and reduce the need for topical corticosteroids in chronic conditions.
