Ceramide synthase 1 (CERS1) is an enzyme and transferase critically involved in sphingolipid biosynthesis, specifically the production of ceramides, with a preference for C18 acyl chains. It plays diverse roles in sphingolipid metabolism, ceramide biosynthesis, and the regulation of cell growth, differentiation, and apoptosis. CERS1 is also implicated in metabolic regulation and mitochondrial quality control. Its dysfunction or altered activity is linked to various disease states, including neurodegenerative diseases, cancer, metabolic diseases (such as insulin resistance and obesity), and epilepsy, making it a significant therapeutic target. Modulating CERS1 activity, for instance through inhibition, can reduce C18 ceramide levels, modulate mitochondrial fatty acid oxidation, and alter apoptosis sensitivity.
