Cerebellin-4 (**CBLN4**) is a secreted glycoprotein and a member of the cerebellin family, structurally containing C1q domains[3][4]. CBLN4 is primarily recognized for its role as a modular synaptic organizer, exhibiting context-dependent modulation of neural circuit formation and synaptic plasticity through weak binding to neurexins and specific interaction with the postsynaptic netrin receptor neogenin-1 (Neo1)[3][5]. Its synaptic organization properties are distinct from those of other cerebellins such as CBLN1 and CBLN2, as CBLN4 shows reduced affinity for neurexin/δ2-type glutamate receptor interactions[3][5]. Outside of the nervous system, CBLN4 serves as a paracrine factor downstream of the SRY and SOX9 transcription factors in Sertoli cells, suggesting a role in testis determination and the initiation of early gonadal development programs[1][3]. There is currently no evidence of direct drug interactions, clinical biomarkers, or well-established safety concerns for this molecule, and it is not widely considered a direct therapeutic target, but rather a regulatory modulator of neurodevelopmental and reproductive processes[5].
