Overview

Cerebral cavernous malformations 2 (CCM2), also known as Malcavernin, is a critical scaffold protein essential for maintaining the structural integrity of the vascular endothelium (UniProt Q9BSQ5). It functions within a heterotrimeric complex alongside CCM1 (KRIT1) and CCM3 (PDCD10) to regulate Rho GTPase activity and p38 MAPK signaling, which are vital for endothelial cell-cell junction stability and proper angiogenesis (NCBI Gene: 11335). Loss-of-function mutations in the CCM2 gene result in Cerebral Cavernous Malformations (CCM), a condition characterized by abnormally dilated and fragile capillaries in the brain that are prone to leakage, leading to hemorrhagic stroke and neurological deficits (OMIM: 607929). As a therapeutic target, CCM2 mRNA is the focus of gene restoration strategies, such as mRNA replacement therapy or viral-mediated gene delivery, designed to compensate for haploinsufficiency in affected patients (PubMed: 30643246). By restoring functional CCM2 levels, these therapies aim to normalize the hyperactive RhoA/ROCK signaling pathway that drives vascular permeability and lesion growth in CCM patients (PubMed: 19273613). While no direct mRNA-targeting drugs are currently FDA-approved, experimental strategies using lipid nanoparticles or adeno-associated viruses to deliver functional CCM2 genetic material are under active investigation in preclinical models. Successful intervention at the mRNA level is expected to stabilize endothelial junctions and prevent the formation of new vascular lesions.
