Overview

Cerebral metabolic rate for oxygen (CMRO2) is a physiological index that quantifies the amount of oxygen consumed by the brain tissue per unit of time, typically expressed in micromoles per 100 grams of brain tissue per minute. It serves as a critical indicator of regional and global brain aerobic metabolism and is tightly coupled with neuronal activity and cerebral blood flow (CBF) under normal physiological conditions (StatPearls, 2023). While not a molecular target like a receptor or enzyme, CMRO2 is a key clinical measurement used to assess brain health and the efficacy of neuroprotective strategies. In pathological states such as ischemic stroke or traumatic brain injury, CMRO2 often declines due to tissue damage or is intentionally suppressed by clinicians to minimize secondary injury (PubMed, PMC4359733). Pharmacological agents, particularly general anesthetics and sedatives like propofol and barbiturates, are frequently used to lower CMRO2 to protect the brain during periods of reduced oxygen delivery or elevated intracranial pressure (NIH, 2021). Consequently, CMRO2 is a primary endpoint in neuropharmacology and anesthesiology for monitoring the balance between cerebral oxygen supply and demand.
