Cerumen is a complex mixture of secretions from ceruminous and sebaceous glands, primarily composed of saturated and unsaturated long-chain fatty acids, alcohols, and cholesterol [1]. Its primary biological function is to protect the ear canal by trapping foreign particles and providing a barrier against moisture and infection [2]. In clinical conditions such as cerumen impaction or otitis externa, these materials can cause hearing loss, pain, or serve as a medium for bacterial and fungal growth. Pharmacological targeting of these surfaces involves the use of cerumenolytic agents like carbamide peroxide or surfactants, which act by softening, emulsifying, or oxidizing the debris to facilitate its clearance [3]. These agents do not interact with a specific molecular receptor but rather alter the physical integrity of the debris matrix to restore canal patency or improve the efficacy of topical antibiotics.
Cerumenolytics and surfactants act by chemically disintegrating the lipid and protein components of the debris, or by releasing oxygen to mechanically break up the material, thereby facilitating its removal from the ear canal [1, 3].
