Overview

Cervical extracellular matrix glycosaminoglycans (GAGs) are essential carbohydrate components that regulate the mechanical properties of the cervix throughout pregnancy (Uldbjerg et al., 1983, PubMed). These molecules, including hyaluronan, dermatan sulfate, and heparan sulfate, interact with collagen fibers to maintain cervical structural integrity (Word et al., 2007, Seminars in Reproductive Medicine). During the process of cervical ripening, there is a marked shift in GAG composition, specifically an increase in hyaluronan and a decrease in dermatan sulfate (Ruscheinsky M, et al., 2008, JBC). This shift increases tissue hydration and reorganizes the collagen matrix, leading to the softening and effacement necessary for childbirth (StatPearls, 2023). Clinical interventions for labor induction, such as the administration of prostaglandins like dinoprostone, work by stimulating the synthesis of hyaluronan and the degradation of collagen-binding GAGs (NIH, PubChem). Abnormalities in cervical GAG metabolism are linked to obstetric complications, including preterm birth and cervical insufficiency. Consequently, these molecules serve as both therapeutic targets for labor management and potential biomarkers for predicting delivery timing.
