Overview

Cervical softening and ripening is a *dynamic tissue remodeling process* in which the cervix transitions from a firm, collagen-dense structure to a soft, compliant state that enables dilation during childbirth. This process involves: breakdown and reorganization of extracellular matrix components (especially collagen), infiltration of inflammatory immune cells (notably macrophages and neutrophils), increased expression of enzymes such as matrix metalloproteinases (MMPs), upregulation of inflammatory mediators (such as IL-1, IL-8, nitric oxide, and prostaglandins), and local hormonal changes. Clinical interventions leveraging this process rely on exogenous prostaglandins and mechanical devices to induce or accelerate cervical ripening before labor or gynecologic procedures[1][2].

Mechanism of action

Prostaglandin analogs increase prostaglandin levels, stimulate collagen breakdown, promote local inflammation, and induce uterine contractions[1]. Osmotic dilators cause mechanical stretching of the cervix and secondarily increase local production of prostaglandins, cytokines, and nitric oxide[1]. Nitric oxide donors induce vasodilation and promote inflammatory signaling involved in cervical softening[2].
