The chloride ion concentration gradient refers to the difference in Cl⁻ concentrations between various compartments in the body, such as extracellular fluid, cytosol, and intracellular organelles. This gradient is fundamental for cell signaling, pH balance, maintaining osmotic pressure, and regulating membrane potential. The gradient is established by the activity of transport proteins, such as chloride channels (voltage-gated, ligand-gated), cotransporters (NKCC, KCC), and exchangers. Disruption of the Cl⁻ gradient is associated with a variety of diseases, especially kidney disorders, metabolic alkalosis/acidosis, neurological disorders, and some muscle diseases. Drugs may indirectly modulate chloride gradients by affecting channel or transporter function but do not target the gradient itself.
