Overview

The cholesterol gallstone matrix is an organic, insoluble, gel-like structure found at the core of human cholesterol gallstones. It primarily consists of a high-molecular-weight complex of **mucin (a glycoprotein)** and **bilirubin** and may also contain smaller amounts of proteins such as serum albumin, bile pigments, and inorganic calcium salts [1][2][4]. This matrix is believed to originate from biliary sludge and acts as a scaffold for the crystallization and aggregation of cholesterol, forming the structure of gallstones [1][2][4]. Studies show the matrix is difficult to dissolve; reducing agents such as 2-mercaptoethanol and N-acetylcysteine, as well as the cholesterol solvent monooctanoin, can partially solubilize the matrix and promote gallstone dissolution in vitro, suggesting the matrix plays a key role in stabilizing gallstones and making them refractory to dissolution [1][4][7]. The "cholesterol gallstone matrix" is not a single molecule and does not represent a receptor or enzyme but rather a heterogeneous biochemical scaffold comprised of macromolecular aggregates.
