Choline-containing membrane phospholipids, primarily phosphatidylcholine and sphingomyelin, are essential structural components of the sperm plasma membrane that maintain its integrity and fluidity. These lipids are critical for the physiological processes of sperm capacitation and the acrosome reaction, which are prerequisites for successful fertilization [PMID: 9663611]. In the field of reproductive health, these phospholipids serve as the primary target for surfactant-based spermicides such as Nonoxynol-9 and Octoxynol-9 [PMID: 11113258]. These agents act by intercalating into the lipid bilayer, leading to the physical disruption of the membrane, loss of intracellular contents, and subsequent immobilization or death of the sperm cell. While effective as contraceptives, the non-specific nature of this disruption can also affect the vaginal and cervical epithelium, potentially increasing the risk of infection. Clinical studies have shown that frequent use of such agents can lead to mucosal lesions, which may facilitate the transmission of sexually transmitted pathogens, including HIV [PMID: 12133651].
