Overview

Choline esters are a group of quaternary ammonium compounds that function as direct-acting cholinergic agonists, mimicking the effects of the neurotransmitter acetylcholine (StatPearls, NBK538163). This class includes endogenous acetylcholine as well as synthetic analogs such as bethanechol, carbachol, and methacholine, which vary in their resistance to degradation by cholinesterase enzymes and their selectivity for muscarinic versus nicotinic receptors (PubChem, Choline Esters). Biologically, these compounds facilitate signal transduction across cholinergic synapses, influencing smooth muscle contraction, glandular secretion, and heart rate. In clinical practice, they are employed to manage conditions like glaucoma and non-obstructive urinary retention, or as diagnostic tools in bronchial challenge testing (NIH, PubMed). However, because they are ligands that act upon receptors rather than being the receptors themselves, they are classified as a drug class rather than a therapeutic target. Their use requires careful monitoring due to the risk of systemic cholinergic toxicity, which can manifest as bradycardia, hypotension, and respiratory distress (Wikipedia, Cholinergic Agonist).
