A *cholinergic neuron* is any nerve cell that synthesizes and releases the neurotransmitter **acetylcholine**, playing key roles throughout both central and peripheral nervous systems. In the brain, these cells are crucial for modulating attention, learning/memory processes, arousal states including wakefulness/REM sleep cycles—and their degeneration is strongly implicated in neurodegenerative diseases such as Alzheimer’s disease. Cholinergic neuronal diversity exists across different brain regions with varying morphology and physiological properties; they can be identified by markers such as choline acetyltransferase expression. While essential for normal cognitive function and motor control via neuromuscular junctions peripherally, they themselves are not direct therapeutic targets—instead pharmacological interventions focus on their molecular machinery such as receptors or enzymes involved in ACh metabolism.
Drugs modulate cholinergic signaling via: - Inhibition of acetylcholinesterase to increase synaptic ACh levels (e.g., donepezil). - Agonism/antagonism at muscarinic or nicotinic acetylcholine receptors. - Modulation of presynaptic release mechanisms.
