Overview

Cholinergic neurons of the basal forebrain (BFCNs) are a diffuse population of neurons, primarily located in the nucleus basalis of Meynert, medial septum, and diagonal band of Broca, that provide the majority of cholinergic innervation to the neocortex and hippocampus (Source: PubMed, PMID: 24703803). These neurons are essential for cognitive functions including attention, arousal, and the encoding of new memories by modulating the excitability of target circuits (Source: StatPearls, NBK535367). In Alzheimer's disease and related dementias, BFCNs undergo early and profound degeneration, which correlates strongly with the severity of cognitive decline (Source: NIH, Alzheimer's Association). Therapeutic interventions traditionally focus on the cholinergic hypothesis, utilizing acetylcholinesterase inhibitors like donepezil to increase the availability of acetylcholine at remaining synapses (Source: PubChem). While BFCNs are a cellular system rather than a single molecular target, they contain specific proteins like the p75 neurotrophin receptor and TrkA that are investigated as targets for neuroprotective therapies (Source: UniProt, P08138).

Mechanism of action

Enhancement of acetylcholine levels via inhibition of acetylcholinesterase (AChE) or butyrylcholinesterase (BuChE) to compensate for neuronal loss; modulation of nicotinic and muscarinic receptors.
