Chromosome 11 open reading frame 58 (C11orf58), also called Small acidic protein (SMAP), is a conserved human protein whose biological function remains largely unknown. Evidence from genetic and bioinformatic studies associates polymorphisms in the C11orf58 gene with increased risk of ischemic stroke, inflammation, and vascular remodeling processes[3][6]. It belongs to the SMAP family and is sometimes referred to by the "Hero20" alias, reflecting probable chaperone-like properties, protection from protein denaturation, and stress response roles[3]. C11orf58 is expressed in brain tissue, blood vessels, and blood[3], but there is currently no evidence it acts as a traditional receptor, enzyme, or transporter, nor is it targeted by therapeutic drugs[6]. The protein is evolutionarily conserved, but detailed molecular pathways and clinical functions await further experimental elucidation[4][5][6].
