Overview

The Ciliary neurotrophic factor receptor (CNTFR) complex is a tripartite signaling unit composed of the CNTFR-alpha subunit, gp130, and the leukemia inhibitory factor receptor (LIFR) (Davis et al., 1993; Stahl et al., 1994). In the retina, these complexes are specifically localized on Müller glia and photoreceptors, where they orchestrate critical neuroprotective responses (Beltran et al., 2007). Activation of the CNTFR complex by its ligand, CNTF, triggers the JAK/STAT3 signaling pathway, which is vital for the survival of photoreceptors under various stress conditions (Wen et al., 2012). In Müller cells, CNTFR signaling induces a reactive state that promotes the secretion of additional trophic factors, creating a supportive environment for retinal neurons (Rhee et al., 2013). This receptor complex is a primary therapeutic target for retinal degenerative diseases, including retinitis pigmentosa and macular degeneration (Birch et al., 2011). Clinical approaches, such as the NT-501 encapsulated cell therapy, aim to provide sustained activation of this complex to slow vision loss (Kauper et al., 2012). However, prolonged activation can lead to side effects like retinal thinning and reduced rod sensitivity, requiring careful therapeutic monitoring (Lipinski et al., 2015).

Agonism of the CNTFR complex triggers the JAK/STAT, MAPK/ERK, and PI3K/AKT signaling pathways, leading to the upregulation of neuroprotective factors and enhanced survival of retinal neurons.
