CKLF-like MARVEL transmembrane domain-containing protein 5 (CMTM5) is a tetraspan-transmembrane protein encoded by the CMTM5 gene on chromosome 14q11.2[1][6]. It is part of the chemokine-like factor (CKLF) superfamily, with structural similarity to both chemokines and transmembrane 4 superfamily proteins[6]. CMTM5 is expressed in various tissues and has several splice isoforms, with CMTM5-v1 being the most studied[1]. In the immune system, CMTM5 is implicated in the regulation of autoimmune responses, with gene methylation changes linked to diseases such as systemic lupus erythematosus and rheumatoid arthritis[1]. It also acts as a tumor suppressor in various cancers, where its downregulation correlates with increased proliferation, migration, and poor prognosis[1][3]. In the cardiovascular system, CMTM5 expression is associated with coronary artery disease and stent restenosis, potentially via effects on endothelial cell behavior[1]. Within the nervous system, CMTM5 is enriched in oligodendrocytes and CNS myelin, where it is essential for maintaining axonal integrity but is not required for myelin formation or structure[2][4]. Decreased CMTM5 is observed in multiple sclerosis lesions, linking it to neurodegeneration[4]. While CMTM5 is a promising therapeutic and biomarker candidate, there are no established drugs directly targeting it, and its precise mechanisms of action remain under investigation.
