Class A basic helix-loop-helix protein 15 (BHLHA15, commonly known as MIST1) is a transcription factor encoded by the BHLHA15 gene[1][3][6]. Its protein structure allows binding to E-box DNA motifs as a homodimer or as a heterodimer with E-proteins such as E12 and E47[2][3]. MIST1 regulates the organization and identity of secretory cells in exocrine tissues such as the pancreas, salivary glands, and stomach, is critical for proper localization of organelles, and plays a role in cell-derived responses to ER stress[1][3][5]. Loss or genetic knockout of MIST1 leads to disorganization of secretory granules, increased cell stress, and heightened risk of pancreatic and liver damage, including cancer[1][5]. MIST1 expression is tightly regulated in development and can serve as a biomarker for cellular injury in metabolic and alcohol-related liver diseases[5]. There is currently no information suggesting direct pharmacological targeting or therapeutic safety concerns related to drugs interacting with MIST1.
