Coagulation factor IX (FIX) and Coagulation factor X (FX) are the primary proteins involved in the intrinsic tenase complex of the coagulation cascade [1, 2]. In a healthy physiological state, activated Factor VIII (FVIIIa) acts as a non-enzymatic cofactor that bridges activated FIX (FIXa) and FX, significantly accelerating the activation of FX to FXa [3, 5]. This process is critical for the burst of thrombin generation required for stable fibrin clot formation [3]. In Hemophilia A, the absence or deficiency of FVIII prevents this bridging, leading to severe bleeding diatheses [4]. Therapeutic strategies, most notably bispecific antibodies like emicizumab, target FIX/FIXa and FX/FXa simultaneously to mimic the scaffolding function of FVIIIa [5, 6]. By bringing FIXa and FX into the correct spatial orientation, these mimetic agents restore the coagulation process independently of FVIII [6, 7]. This mechanism is particularly valuable for patients who have developed inhibitory antibodies against FVIII replacement therapies [4, 7]. Monitoring the efficacy of drugs targeting this complex often requires specialized assays, as standard coagulation tests may be misleadingly affected by the presence of the mimetic [7].
