Overview

Cobalt ions are essential trace elements that serve as the core metallic component of cobalamin, also known as Vitamin B12 (NIH ODS, 2024). In this role, cobalt is indispensable for critical biological processes including DNA synthesis, fatty acid metabolism, and the formation of red blood cells. Beyond its role in Vitamin B12, cobalt ions can act as hypoxia mimetics by stabilizing hypoxia-inducible factor 1-alpha (HIF-1α) through the inhibition of prolyl hydroxylase enzymes (PubChem CID 104729). While essential in minute quantities, elevated levels of cobalt ions are associated with significant systemic toxicity, leading to conditions such as cobalt-induced cardiomyopathy, polycythemia, and hard metal lung disease (StatPearls, 2023). In clinical practice, cobalt ions are the primary target of chelating agents like edetate calcium disodium and penicillamine, which bind the ions to facilitate their renal excretion during cases of poisoning. Additionally, cobalt-containing compounds are used therapeutically to treat vitamin deficiencies and have been investigated for their potential to stimulate erythropoiesis (IARC, 2023).
